I understand that all services and events offered by Sacred Space Wellness Inc. (including but not limited to yoga, meditation, Reiki, sound healing, breathwork, intuitive mentoring, energy work, and life or business guidance) are intended for personal growth, relaxation, and spiritual well-being.

I acknowledge that these sessions do not constitute medical treatment, psychological therapy, or professional health care, and that the facilitator does not diagnose, prescribe, or treat specific physical or mental health conditions. Any recommendations regarding lifestyle, herbal, or dietary choices are for informational purposes only and are not a substitute for advice from a qualified health professional.

I understand that experiences with energy work, intuitive guidance, or sound healing may bring emotional release or temporary discomfort as part of the healing process. I take full responsibility for my own health, choices, and well-being before, during, and after participation in any session, class, or event.

I understand that outdoor sessions may involve walking on uneven terrain, exposure to weather, insects, or natural elements. I accept full responsibility for my comfort, safety, and participation during nature-based sessions.

I will inform the facilitator of any physical conditions, limitations, or sensitivities that could affect my participation.

I understand that cacao is a plant medicine containing natural stimulants (including theobromine and caffeine) and should be consumed mindfully.

I have disclosed any allergies, sensitivities, or health conditions (such as heart issues, high blood pressure, anxiety, or medication use including antidepressants/MAOIs) that may interact with cacao.

I acknowledge that cacao is offered ceremonially for heart connection and personal reflection and is not a medical or therapeutic treatment.

By submitting this form electronically, I voluntarily release and hold harmless Sacred Space Wellness Inc., its facilitators, and affiliates from any and all liability for injuries, losses, or unfavorable outcomes that may result from participation.
